Phaco-trabeculectomy

Phaco-trabeculectomy is a combined surgical procedure that addresses both cataracts and glaucoma. This dual approach involves performing phacoemulsification (a modern cataract surgery technique) and trabeculectomy (a glaucoma surgery) simultaneously. This procedure is beneficial for patients who have both conditions, offering a solution to improve vision by removing the cataract and lowering intraocular pressure (IOP) by creating a new drainage pathway for the aqueous humor.

Components of Phaco-trabeculectomy

1. Phacoemulsification:

  • Procedure: Uses ultrasound energy to emulsify and remove the cloudy lens (cataract) through a small incision. An artificial intraocular lens (IOL) is then implanted.
  • Benefits: Minimally invasive with a quick recovery time and significant improvement in vision.

2. Trabeculectomy:

  • Procedure: Involves creating a small flap in the sclera (white part of the eye) and a drainage hole under this flap to allow aqueous humor to drain from the eye, thereby reducing intraocular pressure.
  • Benefits: Effective in lowering IOP, which helps to manage glaucoma and prevent further optic nerve damage.

Indications for Phaco-trabeculectomy

  • Patients with Coexisting Cataract and Glaucoma: When both conditions need surgical intervention.
  • High IOP: When medical management of glaucoma is insufficient to control intraocular pressure.
  • Progressive Visual Field Loss: Due to glaucoma that requires surgical intervention.

Benefits of Phaco-trabeculectomy

  • Single Surgical Procedure: Combining cataract and glaucoma surgeries into one reduces the need for separate surgeries and anesthesia sessions.
  • Improved Vision: Cataract removal improves visual acuity, while glaucoma surgery helps preserve the remaining vision by controlling IOP.
  • Better IOP Control: Trabeculectomy is effective in significantly lowering IOP, reducing the dependence on glaucoma medications.
  • Reduced Surgical Risks: Performing both procedures simultaneously can reduce the overall risk associated with multiple surgeries.
  • Enhanced Quality of Life: Patients benefit from improved vision and better glaucoma management, enhancing their overall quality of life.
